The picture behind by is right beside. "Sit by the window" puts you next to it. From there the word came to mark whatever is working right alongside the action: its means or its agent. Positional by means near without touching: alongside, with a little space in between. That sense of proximity carries into the means and agent uses. よくある間違い / Common Mistake

受動態の行為者に from を使った形です。from は出どころを示す語で、動作をした主体を示しません。却下したのが委員会なら by。from を使うと「委員会のほうから来た」という別の意味に読まれます。判定は、能動態に戻してみること。The committee rejected the proposal. で委員会が主語になるなら、受動態では by の後ろに来ます。行為者は by、出どころは from です。
This marks the agent of a passive with from. From names a source, not the doer of an action. If the committee did the rejecting, it takes by. With from, the sentence reads as something arriving from the committee instead. The test is to rebuild the active: "The committee rejected the proposal" puts the committee in the subject slot, which means it goes behind by in the passive. Agents take by; sources take from.
