Tom broke the window. から Tom だけを抜き出して枠にはめると、It was Tom who broke the window.(壊したのはトムだ)になります。強調したい要素を It is と that のあいだに置き、残りを that 以下に流す。これだけの仕組みです。抜き出した要素は枠の外に出したので、that 以下には二度と置きません。
Take "Tom broke the window," lift Tom out, and drop him into a frame: "It was Tom who broke the window." You put the element you want to spotlight between It is and that, and the rest of the sentence flows after. That's the whole mechanism. Since the element has moved outside the frame, it never appears again inside it.

Lift It Out, Then Frame It
この it に意味はありません。強調の枠を組み立てるためだけの語です。手順は2つ。強調したい要素を文から抜き出して It is / It was の後ろへ置く。残った部分を that(人なら who、モノなら which も可)でつなぐ。強調できるのは名詞や副詞句で、動詞は入れられません。動詞を強調したいときは別の型を使います。
This it carries no meaning. It exists only to build the frame. Two steps: pull the element you want out of the sentence and set it after It is or It was, then attach the remainder with that, or who for a person and which for a thing. Nouns and adverbial phrases can go in the slot; verbs can't. For a verb you reach for a different pattern.

抜き出した要素に光を当てる。「他ではなくこれだ」という対比が生まれる。Spotlights the lifted element and creates a contrast: this one, not another.

よくある間違い / Common Mistake

窓を割ったのはトムだ。
抜き出した要素を、元の場所にも残してしまった形です。Tom は枠の中へ移したので、who の後ろにはもう主語がありません。he を置くと主語が二重になります。組み立てるときは、元の文から要素を物理的に取り除くイメージを持ってください。Tom broke the window. から Tom を抜けば、残るのは broke the window。これを who に続ければ完成です。
The lifted element has been left behind as well. Tom moved into the frame, so nothing else fills the subject slot after who, and adding he doubles it. When you build one, picture physically removing the element from the original sentence. Take Tom out of "Tom broke the window" and what remains is broke the window, which attaches straight to who.
It was not until ... that ...〜して初めて…した否定を枠に入れる定型。書き言葉で頻出。The set way to frame a negative. Common in writing.
It is you who ...〜なのはあなたのほうだ人称代名詞も枠に入る。反論や指摘の場面で使う。Pronouns fit the slot too, typically when pushing back or pointing out.
What I need is ...私が必要なのは〜だwhat で始めるもうひとつの強調型。動詞や動作も強調できる。The other emphasis frame, the one that can spotlight an action.
All I want is ...私が欲しいのはただ〜だけall を使って範囲を絞る型。「それだけでいい」という含み。Narrows the field with all, implying that nothing more is asked.
まとめ / Summary
強調したい要素を It is と that のあいだに置く。Put the element you want to spotlight between It is and that.
抜き出した要素を that 以下に残さない。Don't leave a copy of the lifted element inside the frame.
名詞と副詞句は入るが、動詞は入らない。Nouns and adverbials fit the slot; verbs don't.
